Waiver and Photo Release: I, the undersigned, hereby acknowledge that I am fully aware of and understand the risks involved in participating in high-risk activities through ChAmp Volleyball Camps. I understand that my child must have their own medical insurance policy in effect during my child’s participation. I hereby authorize the staff of ChAmp Volleyball Camps to act for me according to their best judgement in any emergency requiring medical attention, and I hereby waive and release ChAmp Volleyball Camps, its directors, staff any and all parties, groups, organizations, volunteers, sponsors, and (or) any representatives involved in ChAmp Volleyball sessions from any liability for any injury or illness occurred while at a ChAmp Volleyball Camp. I have no knowledge of any physical impairment that would affect my child in a ChAmp Volleyball Camp program. I give permission and consent to allow photographs to be taken during camp session activities. I further give permission and consent that any such photographs may be published and used by ChAmp Volleyball Camps to illustrate and promote the camp experience, and it's camp programs. *
